I am in-network with Medical Mutual and will submit claims to them on your behalf. If you have different insurance I can provide you with a form that you can send into your insurance company. That means that you’re responsible for the fee at the time of our appointment and, depending on your plan, a certain percentage of that may be paid to you by your insurance company. I would suggest calling them before we meet to clarify what your ‘out of network’ coverage is.
